A dedicated Social Worker committed to supporting vulnerable and older individuals during their transition from hospital. You thrive in an acute, multi-disciplinary setting and are passionate about implementing the Care Act 2014 to ensure comprehensive care and safeguarding.
What the job involves
This role is based within the Hospital Discharge Service, working with clients across the London Borough of Hammersmith & Fulham, Royal Borough of Kensington and Chelsea, and Westminster City Council. You will manage a diverse and complex caseload, perform timely assessments, develop support plans, facilitate hospital discharges, and undertake safeguarding investigations. Working closely with customers, carers, and other professionals, you will help minimize risks while supporting customers to maintain their chosen lifestyle. You will also participate in service developments, appraisals, audits, practice evaluations, and urgent response/duty systems.
