Who You Are
An experienced Occupational Therapist with a passion for delivering specialist assessments and interventions to facilitate safe and effective hospital discharges. You are committed to maximizing independence for residents and supporting system flow within adult social care.
What the job involves
You will work as part of the Hospital Discharge Team to reduce hospital delays and prevent readmissions by conducting Care Act assessments and prioritizing urgent referrals. Your role includes contributing to the Discharge to Assess (D2A) pathway, providing hospital duty cover, and collaborating with NHS colleagues and social care workers to meet residents' needs effectively. You will also be responsible for developing support plans and delivering training to promote strengths-based interventions.
