Who You Are
A Social Worker responsible for assessing care needs and facilitating supported transitions from hospital. You will work within a dynamic, multidisciplinary team across various London boroughs. You'll focus on implementing the Care Act 2014, conducting safeguarding investigations, and managing a varied caseload.
What the Job Involves
As a Social Worker in the Hospital Discharge Service, you will:
- Support vulnerable and older people in transitioning from hospital.
- Carry a varied and complex caseload, ensuring timely assessments.
- Develop support plans and facilitate hospital discharges.
- Conduct safeguarding investigations and adhere to council policies.
- Participate in an urgent response/duty system as directed.
- Work with customers, carers, and professionals to manage risk and enable preferred lifestyles.
- Engage in service development, appraisals, audits, and practice evaluations.
- Maintain up-to-date DBS and professional registration.
- Commit to continuous development through training and reflective practice.
