Who You Are
As a Step-Down Care Manager (Independent Living Assessor), you will be a self-starter and a problem solver, bringing new ideas and energy to a well-established and professional team. You will have at least 1 year of post-qualifying experience in a Local Authority or Health setting, with knowledge of relevant legislation, and a fundamental understanding of Personalisation, independence, and choice. Your experience should include working with older or physically disabled people, Safeguarding Adults legislation, and assessment work, among other critical responsibilities.
What the job involves
In this role, you will manage a varied caseload, ensuring effective risk management and smooth operations within the Step-Down team, focusing on adult social care services. Responsibilities include undertaking timely Care Act assessments and support plans, safeguarding investigations, Mental Capacity Act assessments, and ensuring the Care Act 2014 is implemented. You will work closely with multidisciplinary teams, providing support to minimize and manage risk while enabling customers to maintain their lifestyle choices. Participation in audit, training, and service development is also expected.
