Who You Are
An experienced, passionate, and dedicated advanced social worker practitioner (ASWP) looking to join an Urgent and Emergency Care Team, covering hospitals in the South of Nottinghamshire.
What the job involves
The ASWP will work in a team environment, understanding the Care Act 2014, the Mental Capacity Act 2005, and Department of Health Discharge to Assess guidance. The role involves managing various hospital discharge pathways, addressing delayed transfer of care impacts, and demonstrating proactivity and partnership working. Responsibilities include supporting management to enhance social work practice, driving performance, coaching, and strength-based practice. The ASWP needs to attend, chair meetings with health colleagues, and deputize for the Team Manager as needed.
