Who You Are
An experienced, passionate, and dedicated advanced social worker practitioner (ASWP) with expertise in urgent and emergency care. You have a solid understanding of the Care Act 2014, the Mental Capacity Act 2005, and discharge guidance. You possess strong management and leadership skills and are committed to improving social work practice.
What the job involves
Your role will involve joining the Urgent and Emergency Care Team, covering hospitals in South Nottinghamshire. You will manage pathways for patients leaving hospitals, understand the effects of delayed transfers, and work proactively to solve problems. You will also engage in partnership working, coach others, and support the management team in enhancing social work practices.
