Who You Are
An experienced, passionate, and dedicated Advanced Social Worker Practitioner (ASWP) is sought to join the Urgent and Emergency Care Team in Nottinghamshire. Candidates should have strong familiarity with the Care Act 2014, the Mental Capacity Act 2005, and Department of Health guidance on discharge. Understanding of hospital discharge pathways and the implications of delayed transfers of care is essential. Proactivity, partnership working, and effective problem-solving skills are key.
What the job involves
The ASWP will contribute to the care team across hospital settings and demonstrate leadership in social work practice improvement. Responsibilities include managing team performance, applying strength-based practices, and coaching. Effective communication and decision-making are critical for supporting both patients and the management team. The role also requires chairing meetings with health colleagues and stepping in for the Team Manager when needed.
