What the job involves

This is an exciting opportunity to be based within a local authority integrated health and social care service. The Care Coordinator post will sit within the social work team. The role entails face to face work with service users, their families, and external partners. The purpose of the Care Coordinator role is to identify whether support packages continue to meet the needs of service users and their families. The role will require the successful candidates to make recommendations regarding support packages and arrange and amend support to ensure needs are being met in a person centred and strength-based way. The Care Coordinator will be required to attend multi-disciplinary meetings, present cases, and communicate effectively with service users, their families, and a range of internal and external colleagues and partners.

• Excellent communication skills, both verbal and written
• Ability to write accurate, comprehensive support plans and assessments
