- Must be a registered Social Worker with Social Work England and hold a degree in Social Care or equivalent.

- Ability to communicate complex social issues clearly to both clients and professionals.
- Ability to plan and coordinate social care plans and awareness of the complex aspects of patient discharge.
What the job involves
As part of the Mental Health Social Care Team at the London Borough of Sutton, the role involves coordinating and fulfilling the Council’s statutory social care responsibilities for individuals with complex mental health needs. The focus is to undertake Care Act responsibilities for those with mental health needs within Sutton. The service functions using hub models, focusing on complex long-term cases, reviews, hospital discharge work, dual diagnosis, and an Approved Mental Health Professional hub. The role requires working closely with health colleagues to fulfil S117 functions and facilitate timely discharges from psychiatric wards for Sutton residents. The position offers an opportunity to join the Complex Case hub, working on Court of Protection cases, Safeguarding, and Care Act assessments, or the Review hub, focusing on S117 and Care Act reviews both in and out of the borough.
