We are seeking an experienced Social Worker to join the Lambeth Hospital Social Work Team. You must be adaptable to a demanding environment, committed to partnership and collaborative working, and have experience with adults over 18 years with a range of physical health disabilities and vulnerabilities. A robust understanding of Safeguarding procedures is essential.

What the job involves

The primary responsibility of this role is to deliver strength-based preventative support, assess, and establish supports and safeguard patients. You will also undertake reviews of existing care packages and considerations of placements, refer individuals to reablement, and ensure safe and timely decisions for facilitating discharges. The role involves working closely with MDT colleagues, voluntary sector organisations, and wider agencies to support residents.
