What the job involves
As an Assessor in the Adult Social Care Department, you will undertake assessments and support planning aimed at maintaining the independence of service users. This involves home visits, identifying resources, co-ordinating care, recommending equipment and adaptations, and performing reviews. You must maintain detailed records, manage workloads, and work closely with healthcare providers and agencies to ensure comprehensive care for service users.
