What the job involves
The role involves supporting the Hospital Discharge Team by coordinating care and discharge planning for adults in hospital settings. You will undertake Care Act-related interventions, Mental Capacity Act work, and safeguarding activities. The SWO plays a pivotal role in patient flow, emergency, and planned discharges, working alongside multidisciplinary partners to ensure patients are discharged to suitable settings with the necessary support. Responsibilities include conducting Care Act assessments, facilitating referrals and services, supporting safeguarding and mental capacity processes, and contributing to the hospital's discharge initiatives.
