Who You Are
To be a good fit, you’ll bring at least one year post-qualifying experience in a Local Authority or Healthy setting, with a Social Services Duty System. You have experience of working with older or physically disabled people and have completed timely, accurate assessment work. You are familiar with the effectiveness of a multi-disciplinary approach, are good at clear communication, both verbally and in writing, and have good computer literacy. You have a fundamental understanding and commitment to the principles of personalisation, independence, and choice.
What the job involves
As a Social Worker in the Hospital Discharge Service, you will assess the care needs of vulnerable and older people, ensuring supported transitions from hospital. You will carry a varied caseload and ensure the implementation of the Care Act 2014. Your role involves timely assessments, developing support plans, safeguarding investigations, and participating in an urgent response/duty system. You will work with customers, carers, and other professionals to manage risk and will participate in service developments, trainings, and reflective practice.
