What the Job Involves
As part of the Community Active Recovery Team (CART), you will assist residents new to Adult Social Care. Your role involves completing compliant assessments under the Care Act 2014 with an independence-first approach, engaging with various stakeholders to develop person-led Support Plans, and collaborating with multi-disciplinary teams to ensure residents live independently. You'll manage reablement support for up to six weeks, identify safeguarding concerns, and deliver high-quality social support.
