What the job involves
In this role, you will shape pathways and collaborate across Health and Care to promote residents' independence and wellbeing. You'll work in a fast-paced environment connecting people with effective solutions and empowering them to thrive. You'll implement Occupational Therapy best practices, champion person-centered approaches, identify residents for early intervention, complete assessments, and collaborate with multidisciplinary teams to design comprehensive care plans.
