Who You Are
The New Malden Hub is looking for a Social Worker with an undergraduate or postgraduate degree in Social Work, registered with Social Work England, and holding the legal right to work in the UK. The ideal candidate will have excellent communication skills, both in written and oral presentation, and possess good assessment and case management skills. Additionally, knowledge of Personal Budgets/Self Directed Support and the related philosophies, as well as a sound understanding of relevant legislation and social care initiatives, is required.
What the job involves
The role involves facilitating effective assessment and support planning in accordance with the Care Act 2014 and the Mental Capacity Act 2005 for adults with physical health needs. Responsibilities include undertaking service user reviews within statutory timescales, managing a number of cases including safeguarding investigations, and communicating with health and social care colleagues for efficient support planning. The role also involves ensuring appropriate referrals, considering funding paths, and making effective use of resources through strength-based care planning.
