A motivated and experienced Social Worker committed to promoting independence, safeguarding vulnerable adults, and delivering person-centred care. Confident working in a multi-disciplinary environment and supporting service improvement and professional development.

What the job involves

This role is responsible for supporting adults with complex needs by carrying out assessments, developing support plans, and coordinating services that promote independence, wellbeing, and positive outcomes. The post holder works within a strength-based, person-centred approach, helping individuals make informed choices while managing risks and safeguarding concerns. Main responsibilities include managing a complex caseload, completing assessments and reviews, identifying opportunities for reablement, undertaking safeguarding enquiries, supporting discharge planning, providing advice and guidance, and contributing to mentoring and practice development.

Skills

  • Qualified Social Worker registered with Social Work England
  • At least two years' post-qualification experience in adult social care
  • Strong knowledge of adult social care legislation and safeguarding
  • Ability to work independently and manage competing priorities
  • Excellent communication and problem-solving skills
  • Ideally hospital social work experience
