You have experience working in the NHS or an integrated commissioning function. Your background includes managing NHS Continuing Health Care (CHC) teams and Better Care Fund (BCF) initiatives. You possess the ability to build working relationships and collaboration across different stakeholders such as the council and the wider health system.
What the Job Involves
This role is a significant part of the commissioning team funded by Southwark Council and SEL ICS, focusing on older and/or frail residents in Southwark. You will lead and contribute to specified programmes of work, play a vital role in professional development within the team, and coordinate complex health and care programmes. You will assess current commissioning arrangements, engage with various stakeholders, and develop a commissioning plan to reduce health and wellbeing inequalities. Your responsibilities include sustaining relationships with the NHS, voluntary sector, local partners, and service providers, with occasional evening and weekend meetings required.
