Who You Are

We are looking for a proactive and compassionate Assertive Move-On Worker to join a busy Housing Support Team, helping individuals move from supported accommodation and temporary accommodation into sustainable, long-term housing. You should be passionate about supporting people to achieve independence and secure sustainable housing outcomes.

What the job involves

This rewarding role focuses on preventing repeat homelessness by working directly with people who may face complex barriers to housing. You will provide practical support, coordinate with partner agencies, and help service users develop the skills and confidence needed to maintain successful tenancies. Key responsibilities include:

  • Support individuals to move from supported and temporary accommodation into suitable long-term housing.
  • Complete housing support and risk assessments and develop person-centred support plans.
  • Work alongside housing providers, support services, health professionals and other partner agencies to achieve positive outcomes.
  • Assist service users to understand housing processes, housing options and tenancy responsibilities.
  • Provide advice and support around budgeting, welfare benefits, debt, income maximisation and tenancy sustainment.
  • Advocate on behalf of service users and support access to appropriate services and community resources.
  • Contribute to move-on planning, nominations and housing pathway processes.
  • Deliver guidance and training to support staff and providers on move-on and resettlement practices.
  • Maintain accurate case records, reports and outcome information.
