Who You Are
You are a qualified social worker with a strong understanding of care act assessments, mental capacity assessments, and safeguarding work. You have experience with the MOSAIC system and can start on 24th August 2026. You must be able to drive and have access to a vehicle.
What the job involves
Based at Mansfield Community Hospital, you will work in a hybrid model as part of the Urgent and Emergency Care Teams. Your role involves supporting hospital discharges, managing complex caseloads, planning and reviewing individual needs, and promoting independence. You will be responsible for conducting assessments, identifying reablement opportunities, liaising with local providers, and undertaking safeguarding work. Additional responsibilities include contributing to practice development and mentoring junior staff.
