What the Job Involves
This role involves delivering high-quality social care support to adults experiencing mental ill health. You will support residents to achieve positive outcomes by working closely with health partners, community services, and internal teams. Key responsibilities include participating in the daily duty rota, undertaking Care Act strengths-based assessments, completing care and support plans, and managing funding applications. You will also work jointly with health colleagues, plan safe discharges from hospitals, and act as a Lead Enquiry Officer for safeguarding enquiries.
Skills
- Social Work England registration
- Experience in community mental health teams
- Understanding of Care Act and Mental Capacity Act
- Ability to work with multidisciplinary teams
- Strong organizational skills
- Experience with multidisciplinary working
- Experience with older adults and complex mental health issues
- Proficiency in IT systems like Mosaic
- Good legal knowledge of relevant Acts
- Flexibility and enthusiasm
