What the Job Involves
This role is within Wrexham Children Services, specifically with the Leaving Care Team, working with care-experienced children and young people aged 15-25. The focus is on supporting transition planning to independence, effective assessment, targeted support, and intervention. You will work collaboratively to ensure positive outcomes, promote safety, and help young people develop life skills and confidence. Responsibilities include providing support for education, employment, training, and ensuring accommodation needs meet young people's preferences.
