What the Job Involves
As a Personal Advisor for Southwark Council's Care Leavers Service, you will support and empower young people in care and those who have left care, helping them transition to independent living. Your responsibilities include assessing young adults' needs, managing a caseload, and prioritizing tasks. You will create, implement, and review pathway plans, attend meetings, and maintain strong relationships with key agencies to ensure a coordinated response. You will also produce reports and maintain records while considering the diverse needs of care leavers.
