A dedicated and experienced social worker with a background in Adult Social Care and specific experience working with individuals with learning disabilities. You are empathetic, self-reliant, and resilient, capable of managing a demanding workload independently while maintaining professional standards.
What the job involves
This role primarily involves conducting comprehensive Care Act reviews and reassessments for adults with learning disabilities. You will undertake face-to-face visits to gather evidence, assess care and support needs, and produce high-quality assessments and reports. The job requires collaboration with a variety of stakeholders to ensure that support arrangements are person-centred and outcome-focused. Participation in multidisciplinary discussions and decision-making processes is essential, with a strong emphasis on promoting equality, diversity, and inclusion.
