Outreach Support Worker (Scale 5)

Support Workers are required within Outreach to ensure the service can continue to meet the needs of individuals living at Vincent House and provide safe, consistent support. The requirements of the post are to support adults with a learning disability who live in their own homes, helping them to maximise their independence, work towards the agreed outcomes and goals set out in their reviews and support plans, mitigate identified risks, and carry out key worker responsibilities.
We are looking to recruit two candidates for this order. One candidate will work up to 35 hours per week on a four-week rolling rota, working five days per week to include alternate weekends.
The second candidate will work a minimum of 16 hours per week, also to include alternate weekends, with the option to work additional hours to cover staff annual leave and sickness where required. The rota pattern can be discussed further during interview.
