Role

To provide a community based model of social work and identify outcomes for individuals to support the development of progression focused support in accordance with legislation, policies and procedures of the Council and best practice. Develop and maintain joint working arrangements with other professionals and agencies. dentify and understand community resources in the locality of the individual to support the delivery of the outcomes identified in the support plan. Attend meetings, including case conferences and make appropriate professional contributions to the proceedings. Undertake assessments that identify outcomes in partnership with the individual, family, and carer and other professionals. Undertake financial assessments within the charging frameworks of the Council. Identify and jointly agree desired outcomes on the basis of assessed need. Ensure that all social networks are identified so that the outcomes are  delivered. Ensure that all relevant community resources are identified in order that the outcomes are delivered. Identify and co-ordinate the assistance and interventions necessary meet needs and desired outcomes. Specify responsibilities for delivering the care plan to meet needs and desired outcomes. Carry out social work casework appropriate to assessed need. Arrange the provision of services to meet need within the resources available and within the framework of the policies and procedures of the service. Initiate appropriate action to protect service users at risk. Manage a caseload efficiently in line with local expectations, e.g. response framework, Positive Risk Taking Framework. Maintain an electronic calendar so that colleagues and supervisors know, at all times, your location and activity undertaken. Make best use of your time and resources to deliver proportionate and efficient services. Monitor and review care plans as appropriate in accordance with Directorate policies and guidance. Maintain accurate and up to date records that provide evidence of need and risk and give a full explanation of the decisions taken. Complete required documentation within relevant timescales and comply with care management standards. Manage time and prioritise work demands. Contribute to the development of the team and people’s learning, including taking responsibility for personal continuous professional development. Attend and contribute to team meetings. Identify shortfalls in available services and unmet need. Contribute to the monitoring and improvement of services. Be aware of and actively promote the Authority’s equality policies and reflect equality principles in one’s own attitude and behaviour. Assist in the recruitment and selection of staff of appropriate grades. Develop and encourage community enterprise and encourage the utilisation of natural community resources and individual strengths. Create and maintain a healthy, safe and productive working environment undertaking risk assessments as may be required. Respect the right of individuals to privacy, maintaining confidentiality where this is required and appropriate. Identify when it is appropriate and lawful to disclose information related to service users and make necessary disclosures within legal and policy frameworks (GDPR).  Managing a greater balance of more complex cases and provide support to staff in those cases where the risks are high and where interventions are of a complicated nature. Chair reviews, meetings or case conferences. Undertake projects and development work to enhance the case management and related services. Assist in the recruitment and selection of staff of appropriate grades. CHC assessments. Carry out non-criminal investigation as part of the AAR process. Carry out training and responsibilities of a Approved Mental Health Professional if requested as relevant to role.
