Who You Are
You are an experienced Acute Hospital Social Worker with a strong understanding of the discharge planning process and knowledge of the Mental Capacity Act. You are comfortable working onsite daily and are committed to supporting hospital discharges in line with the intermediate care process.

The role involves supporting hospital discharges by utilizing your knowledge of the Mental Capacity Act and discharge planning. You will work onsite daily to ensure the intermediate care process is followed effectively.
