What the Job Involves
As a key team member, you will meet the physical, social, and emotional needs of service users as outlined in their care plans. Your role involves assisting and empowering individuals to maintain their independence, improve daily living skills, and partake in recreational activities. Working from Monday to Friday, from 9 am to 4 pm, you'll liaise with other professionals to minimize risks, support service users in their homes, and contribute to the formulation and implementation of person-centered support plans. You will also document changes in circumstances, attend supervision sessions, and ensure your work complies with statutory, health and safety, and City Council policies.
