Who You Are
The Progression and Next Steps Social Worker is a dedicated professional committed to promoting independence and positive outcomes for adults with learning disabilities, autism, and/or complex needs. You focus on strengths-based practice, support autonomy, wellbeing, and inclusion, and work towards reducing long-term dependency.
What the Job Involves
The role involves undertaking high-quality assessments and care planning, developing person-centred support plans, and collaborating with individuals, families, and providers for progression planning. Engaging in multi-disciplinary working, maintaining high professional standards, participating in practice development, and ensuring safeguarding compliance are key responsibilities.
Skills
- Qualified Social Worker with Social Work England registration
- Strong knowledge of the Care Act 2014 and safeguarding procedures
- Experience with adults having learning disabilities or complex needs
- Ability to undertake clear, analytical assessments
- Strong written and verbal communication skills
- Ability to work autonomously and manage varied caseloads
- Experience in supporting transitions and service improvement
- Understanding of progression-focused models and strengths-based practice
- Commitment to promoting independence and person-centred approach
