What the Job Involves
As a Specialist SEN and Disability Outreach Worker within Enfield's Service for Disabled Children, you will be responsible for supporting children and their families to access person-centered short breaks and community activities. Your role includes providing advice, information, and guidance to families, undertaking assessments and reviews for short breaks, and working alongside the SEND Transition Manager to facilitate timely transition pathways. You will take on a lead professional role when appropriate, ensuring that children and young people's participation and views inform service development. Additionally, you'll collaborate with Social Workers to ensure progression of support plans.
