Who You Are

You are a dedicated professional with a passion for supporting young people transitioning out of care. You are capable of managing multiple responsibilities and are committed to ensuring that young people receive the necessary guidance to prepare them for independence.

What the job involves

Your main role will be to manage a caseload of young people who are preparing to leave care. You will collaborate closely with allocated Pathway Advisors, completing and maintaining pathway plan assessments and initial pathway plans. You will be responsible for these plans until the young person reaches 18 years old, at which point responsibility transfers to the Pathway Advisor. Additionally, you will participate in the duty system to provide extra support to young people.
