What the Job Involves
This role serves as a pivotal point of contact and coordination for young people aged 16 to 25 leaving care. You will lead the coordination of resources from various agencies, ensuring a smooth transition to adulthood. The role requires direct work with young people and their networks to achieve individual plan outcomes. The work involves challenging and supporting young people, their families, and associated agencies to improve outcomes for care leavers. You'll provide targeted interventions, crisis support, and serve in a duty service capacity to deliver guidance and ensure young people's needs are met.
